Forgotten User Name

· Navigate to EXCELSIOR by typing https://excelsior.meritcro.com into your web browser

· Select “Forgot User Name?”

· Enter your e-mail address that was used for EXCELSIOR registration and select “Done”.

· Select “OK” when the email notification confirmation appears

· A notification from noreply@ophthalmictrials.com will be sent to you containing your User Name and a link to the EXCELSIOR login page
